How do parishioners sign up for FORMED with a parish account?

Signing up for FORMED is quick and easy. Just follow the simple instructions below.
- Go to the FORMED SIGNUP PAGE (http://formed.org/signup)
- Search for your parish by Zip Code/ Postal Code. Click on your parish.
- Register with your name and email address
- Check that email account for a link to begin using FORMED
- Please NOTE: We no longer use custom URLs or Parish Codes. The only thing you need is your parish's Zip Code.
